Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. If unanticipated treatment is rendered, it is the responsibility of the patient or their guardian to make all necessary payments in full within 30 days.
Additionally, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account.
We also accept Care Credit, a third-party financing option that offers up to 12 months with no interest. Please see our business staff for an application or go to www.carecredit.com to apply online.
Discounts are available for comprehensive treatment plans of $500 or more, per patient, at the inception of treatment. 3% for credit card transactions and 5% for cash or personal check transactions. We accept the following: VISA, MASTERCARD AND DISCOVER.
Missed appointment fee: We appreciate your consideration in giving at least 48 hours notice if you must reschedule and appointment with our office. If you do not show for an appointment you made, or cancel with less than 48 hours notice, a fee may be charged.
